Step-by-Step Instructions

4-Step Process in Joining Genesis and Enrolling in Health Plans

New providers must complete the following steps to initiate the credentialing process. Please follow step-by-step instructions for details on completing the necessary requirements.

Credentialing Requirements

Log in to your CAQH account and complete the following steps:

  • Authorize Genesis Physicians Group to access your data.
  • Update your CAQH profile with accurate and up-to-date information, including:
    • Professional education and training, including applicable training in the specialty for which you are applying.
    • Work history, including an explanation for any work history gaps greater than six months.
      • Current workplace, including the practice you are being added to through GPG, must be listed in your CAQH work history.
    • Updated practice information. Genesis imports practice locations directly from CAQH, so all practice locations must be listed accurately to avoid credentialing delays.
    • Explanations for any disclosure questions answered “YES.”
  • Upload the required supporting documentation, including:
    • Copy of current Texas State Medical License

    • NP/PA: Nurse Practitioners and Physician Assistants must be supervised by a licensed physician and must be listed under the supervising physician’s license with the Texas Medical Board.
    • Copy of current Texas DEA Certificate, if applicable

    • Copy of current Certificate of Malpractice Insurance, minimum $200,000 per occurrence / $600,000 aggregate.
    • Board Certification, if applicable: must be Board Certified or Board Eligible.
    • Hospital Affiliations, if applicable: must have active hospital privileges. Temporary privileges do not fulfill the requirement.


Physicians:
To be eligible for membership with Genesis Physicians Group, a physician must meet the following Board Certification criteria in his/her primary specialty:

  1. Be Board Certified or have been Board Certified, MOC is not required, in the primary specialty by a board recognized by the American Board of Medical Specialties or the American Osteopathic Association.
  2. Be Board Eligible in the primary specialty, or upon changing primary specialty, and Board Certified within five to seven years after completing the highest level of training in the primary specialty.
  3. Have completed an accredited residency or fellowship that makes the physician eligible to complete Board Certification in the primary specialty within five to seven years of completing that residency.
  4. Specialize in an area that has no board certification but is recognized by ABMS/AOA, and complete an accredited residency or fellowship in that specialty and begin practicing in the specialty within five years of completing the residency or fellowship.


Physician Assistants
must be board certified by the National Commission on Certification of Physician Assistants and maintain certification for continued participation.

 

Nurse Practitioners must be certified by one of the following national certification boards and maintain certification for continued participation:

  • ANCC: American Nurses Credentialing Center, subsidiary of the American Nurses Association.
  • AANPCP: American Academy of Nurse Practitioners Certification Program.
  • AACN: American Association of Critical Care Nurses.
  • PNCB: Pediatric Nursing Certification Board.
  • NCC: National Certification Corporation for Women’s Health Nurse Practitioners.
  • NNP: Neonatal Nurse Practitioner.
  • Hospital Exempt: Allergy/Immunology, Dermatology, Ophthalmology, Pathology, and Radiology.
  • Hospital Coverage Letter: Family Practice, General Practice, Geriatric Medicine, Internal Medicine, Pediatrics, and Physical Medicine.
  • Hospital Privileges: Required for all other specialties. Must have active hospital privileges. Temporary privileges do not fulfill the requirement. Must be an acute care facility. Ambulatory surgery centers will not be accepted.

Allied Health Professionals: Physician Assistants and Nurse Practitioners

(State of Texas requires that Allied Health Professionals have supervising physicians delegating their prescriptive authority. The Allied Health Professionals will be opted into same plans as their supervising physician.) State of Texas PA and APN’s Supervision
